To enter the Sunnybrook Staff Lottery, please complete the below form. By completing this form, you acknowledge and agree to the lottery rules and regulations. Draws take place every two weeks, on the Friday following Sunnybrook pay day.
To be eligible for the lottery you must be one of: an active Sunnybrook employee; an active employee of an eligible third-party employer; or hold an eligible professional licence. Click here for full eligibility information.
